This free app connects your ER doctor with your Heart House physician to inform on the next-best step in your treatment during a cardiac emergency.

This coordination puts the ER doctor one step closer to delivering care specific to you and your unique cardiac needs. Better coordinated care means you won’t need to go through duplicate or unnecessary tests and, when appropriate, avoid hospitalization. You will receive an invitation from The Heath House via text message or email if you are eligible for Corventive Patient.
